Lead Care Coordinator - RN (Specialty)
We are seeking a Lead Care Coordinator (Registered Nurse) to join the Delaware Valley team in a hybrid role! The Lead Care Coordinator, RN serves as a resource to the care coordination team. The Lead Care Coordinator RN assists the Clinical Supervisor of Care Coordination and the Manager of Care Coordination/Community Health Worker by providing clinical oversight and direction that ensures the safety and quality of the care provided to patients and families.
