Dorn VAMC is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. Accurately entering and updating data of Veterans' condition on the Transfer Log, VISN 7 Bed Board, Daily Reports, Electronic Morning Report log etc. Entering clinical information in CPRS Charting Coordinating transfers with VISN 7 facilities during and following VISN 7 Bed Calls Coordinating transfer-in of Veterans from community/non-VAMC hospitals Attending and participating in Bed Huddles, Transitioning Level of Care meetings, phone conferences, collaborative meetings, phone conferences, etc. Prioritize bed assignment in collaboration with Bed Control and clinical staff for patients kept in temporary bed locations for movement to the first available Participates in flow briefings & Hospital Flow Rounds discussion Collaborates with members of the health care team and leadership to identify and alleviate barriers to patient access and movement throughout the Columbia VA Healthcare System. Partners' with community and facility stakeholders to promote clinical access to medical services and supports throughput activities within the Miami VA Healthcare System. Works in conjunction with the Clinical Flow Team Supervisor to accomplish assigned duties.
