The role you’ll contribute:
AH Transplant Institute (AHTI) Clinical Coordinators are members of a multidisciplinary team who work together to provide individualized care to patients who are seeking and/or have received an organ transplant. Responsibilities include coordinating patient care through all phases of transplantation which include pre-operative, peri-operative, and post-operative care and follow up. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all

The following responsibilities and core competencies may vary by organ group
PRE-OPERATIVE:
Facilitates/coordinates individualized evaluations for potential organ recipients or living donors which includes collection of patient medical data, patient/family education, identification of co-morbidities/contraindications to transplant (or living donation), collaboration with transplant team members.
Communicates and collaborates with the appropriate multidisciplinary transplant team members and referring/treating physicians.
Maintains appropriate nursing documentation of evaluation process and progress
Utilizes knowledge/compliance with Organ Procurement Transplant Network and United Network for Organ Sharing. (OPTN/UNOS) policies and requirements
Coordinates care of candidate as needed while patient awaiting transplant or living donation
PERI-OPERATIVE:
Coordinates potential transplant surgery
Communicates with patient, transplant physicians, surgical team, donor team, and hospital departments to facilitate patient admissions for transplant or living donation.
Maintains appropriate documentation of transplant surgery per OPTN/UNOS/AHTI policy
POST-OPERATIVE:
Coordinates patient care in outpatient setting to achieve optimal healthcare recovery
Reinforces patient education
Monitors/assesses patient for potential post-transplant complications
Assesses patient compliance with post-transplant plan of care and medication regimen
BONE MARROW TRANSPLANT:
Coordinates the entry and the integration of patients into the program by facilitating appointments for tests, procedures, and required family services. Coordinates and expedites all BMT protocols and informed consents. Communicates and collaborates with multi-disciplinary team and other healthcare personnel relevant to the program and the status of upcoming patients
Coordinates/schedules the pre-bone marrow transplant evaluation for the patient and donor, when applicable
Coordinates/schedules the post-bone marrow transplant evaluation and testing according to patients’ protocol and in collaboration with attending physicians and office protocol
Performs calendar mapping for transplant conditioning
Communicates with cryopreservation laboratory to coordinate bone marrow harvesting, processing, cryopreservation, and reinfusions
Communicates the schedule for stem cell collections/harvest with the dialysis department
Coordinates financial authorizations for BMT care with PFS, Managed Care, and BMT center administration by communication with insurance provides and case managers
Coordinate with the blood bank to facilitate designated donor and phereses blood products for the patient
Facilitates patient and family education regarding all aspects of GMT care with assistance of primary care nurse. Assists in development of BMT education tools to be used in BMT patient teaching.
Completes NMDP Traxis searches, assistant in locating donors both related and unrelated through HLA typing.
Assists in updating protocols in collaboration with the medical director, protocol nurse, and patient
Assists in data collection for the program and reporting for Managed Care contracts
Participates in activities, hospital committees, and professional organizations relevant to BMT
Provides clinical support when needed to clinical staff nurses and trains new employees for the various duties required of a BMT care coordinator.
Qualifications
The expertise and experiences you’ll need to succeed:
Strong assessment skills
Sound clinical judgment, decision making, and problem-solving skills
Concise oral and written communication skills
Ability to work independently as well as within a team-environment
Effective time management skills in high stress environment
Basic computer skills
Critical care experience or palliative care of patients with life-limiting or life-threatening illness
If in BMT unit – minimum of two years’ experience in field of leukemia, autologous, allogenic, and MUD transplantation; or equivalent combination of education, training, and experience
Current valid State of Florida or multi state license as a Registered Nurse
BLS (Basic Life Support)
If in BMT unit – ACLS (Advanced Cardiac Life Support)
