SUMMARY OF POSITION:

Functions as part of an interdisciplinary team to maintain continuity of patient care for identified patient populations from portal of entry to discharge, utilizes appropriate criteria for determining the appropriate level of care for the patient, offers alternative care settings when appropriate, and care plans for timely discharges. Responsible for coordinating patient's clinical plan of care guided by Patients/Families, Physician Providers, and other members of the interdisciplinary team while adhering to guidelines and contracts from regulatory bodies and insurers.

MINIMUM QUALIFICATION(S):

Bachelor's degree in Nursing

Three (3) years of related experience required. 

Five (5) years of direct care nursing experience required. 

Active license to practice as an RN in the Commonwealth of Pennsylvania required.

PREFERRED QUALIFICATION(S):

Previous experience in case management, care coordination, outcomes management or discharge planning preferred. 

Certification in case management from a nationally recognized organization preferred.
