Your day.

As a Licensed Clinical Social Worker, you need to know how to:

• Conduct psychosocial assessment interviews with patients to approve or deny readiness for bariatric surgery


• Provide triage, assessment, and counseling for patients as identified as needing behavioral health intervention


• Develop and implement treatment plans for stabilization and behavioral modification, mobilization and disposition appropriate for patient care. 


• Function as part of a multidisciplinary team to develop patient care plans


• Provide services using accepted psychotherapeutic models and interventions


• Provide consultation to primary care providers as needed and collaborate with staff and providers on treatment plans.


• Consult and act as liaison with all other members of the treatment team to mobilize patient


• Provide educational information to patients and their families


• Provide aftercare planning


• Participate in patient-centered departmental and inter-disciplinary meetings as well as non-departmental meetings and ongoing hospital committees as requested.


• Maintain clinical skills through continuing education.


• Complete all required competencies and maintain all health standards as required by hospital in a timely manner

Your experience.

• Masterâ™s Degree in Social Work


• Licensed Clinical Social Worker in the State of Colorado  (LCSW)


• Knowledge and experience with managed care

Preferred:

• Spanish proficiency


• Previous experience in integrated healthcare, bariatric or primary care


• Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals


• Experience with PC software applications in word processing, Epic, Google applications
