RN Case Manager, Home Care - $15,000 Sign-On Bonus
United Health Services in Upstate New York is seeking a full-time Registered Nurse Case Manager to join our Home Care team based in Johnson City, New York. The successful candidate will be assigned 20-30 patients across a specific geographic territory. Travel to patients’ homes is required—average expected travel is 25-30 miles per day, for which you will receive mileage reimbursement.

RN Case Manager Position Overview
The RN Case Manager manages the in-home nursing needs of their assigned patients from admission to discharge, ensuring patients receive quality care in a financially responsible manner. RN Case Managers are also responsible for providing skilled nursing services and education in patients’ homes. You’ll work with the Nursing Coordinator and an interdisciplinary team to develop and implement a patient care plan in accordance with their physician’s orders.
Job Responsibilities
- Conduct the initial in-home patient assessment visit and determine whether the patient could benefit from our UHS Home Care program
- Develop a specific patient care plan, through collaboration with other Home Care disciplines and community resources, to foster successful attainment of patient goals and a positive transition from home care to independent management of care needs
- Maintain knowledge of OASIS guidelines with the support of the Quality Manager to ensure the patient’s care needs are accurately reflected in their case matrix
- Provide in-home skilled nursing care and perform ongoing assessments of patient care needs, amending the patient care plan as appropriate
- Teach patients and their support systems the necessary skills and knowledge to foster independent management of their disease process (including comorbidities) and to reduce the risk of re-hospitalization
- Complete all documentation according to Home Care policies, including submission of completed documentation with 24 hours of an in-home visit
- Orient and supervise care provided by Home Health Aides in accordance with Home Care policies; participate in the orientation and training of new employees and nursing students
- Maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs

Position Requirements
- Graduate of an accredited Registered Nurse program or a non-approved program supplemented by content equal to an approved program
- Licensure or license eligibility to practice as a Registered Nurse in New York State
- Preferred: Bachelor of Science in Nursing degree and/or medical-surgical or home care experience
