The VHA Nationally Designated-Clinical Resource Hub (CRH) is a strategic program designed to provide Team Based Primary Care, Specialty Mental Health and other specialty services in each VISN across the Enterprise, utilizing existing and emerging technologies. The CRH RN is responsible for managing care across the health care continuum for a select group of clients. Responsible and accountable for all elements of the nursing process when providing direct patient care. Independently provides professional assessment, coordination, telephone triage and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are received, and that progress is being made. The incumbent is responsible for providing direct patient care to Veterans and training and support to CRH and Spoke site staff as appropriate. Core elements of performance include, but are not limited to the following: Utilizes knowledge and active promotion of evidence-based practice and patient driven care to provide care to patients; Promotes access with virtual or face-to-face visits as appropriate; Utilizes motivational interviewing and Whole Health coaching techniques to guide patients to establish goals; Actively participates in unit level quality improvement processes and initiatives and customer service programs; Acts as a Subject Matter Expert (SME) related to telehealth in support of the CRH and Spoke site staff. Responsibilities for specific nursing roles are listed below. Behavioral Health (BHIP) RN The CRH BHIP RN uses the nursing process and evidence-based practice to provide care to the Veteran utilizing the BHIP model. This individual is responsible for providing Behavioral Health Nursing specific education to CRH Spoke site patients, ensuring that CRH services are completed at the highest standard. The incumbent is a member of the CRH mental health team, providing direct patient care to Veterans. The CRH BHIP RN provides patient and family health education with a focus on mental health self-management, prevention, and wellness, based on the patient's goals. The incumbent provides direct care to patients in the BHIP model and collaborates with other members of the behavioral health care team, to augment provider mental health services, creating a comprehensive approach to virtual outpatient mental health. The incumbent assesses patients and provides additional support between visits with LIPs. The CRH BHIP RN follows standard processes used by the mental health team. Uses data registries to assist in identifying high risk patients for monitoring and follow-up. The position may require travel to provide training and support CRH Spoke sites and possible physical deployment to Spoke sites to provide care to panels of patients for an extended period of time Work Schedule: Monday - Friday, 0700 - 1630, Tours varies Compressed/Flexible: Available Telework: Available Remote: While care is anticipated to be conducted via virtual modalities there is the potential for in-person delivery of services dependent on clinical need.
