Summary

Functions as the key member of the health care team responsible for ensuring appropriate discharge planning ensured through assessment, (social, financial, and health conditions of patient) from which recommendations for referrals, services, and post hospital care will be provided. This function will be in collaboration with physicians, nursing staff, physical therapy and/or other members of the multi- disciplinary team as appropriate.

Qualifications:

Education

Bachelors degree in Social work or related field preferred

Experience

Required:
  • Requires knowledge of State and Federal regulatory agency standards, especially those related to pre-admission and discharge screening/planning

  • Cognitive and technical knowledge and

    ability to progressively manage care delivery across the continuum of care, as well as knowledge of community resources and referral mechanisms is essential

  • The ability to retrieve, communicate and present data and information both verbally and in writing is required

  • Must be flexible with work scheduling including "on-call" coverage

Preferred:

  • Previous experience as Social Worker or case management
