We currently have two positions available for Physicians in our HPH Hospice location in Hudson, FL.
Hospice Staff Physician - Has the responsibility of assisting the Medical Director in the care and treatment of patients served by the company. S/he provides physician direction and guidance to the company's program and staff to assure the maintenance of quality medical care for patients and families.
Medical Director - The affiliate Medical Director (MED) has overall responsibility for the medical direction of the care and treatment of patients served by its program's care teams. S/he provides physician direction and guidance to the hospice and staff to ensure the maintenance of quality care for patients and families. The MED serves as liaison between the program and community physicians, as well as with others engaged in health care services. S/he serves as an educator of hospice care. S/he or his/her designee provides direct medical care, as needed, to patients in the home and in multiple health care settings. The Medical Director supervises and manages the medical staff and contract physicians in the specified service area.
Hospice Staff Physician Qualifications and Responsibilities:
MD or DO from an accredited medical school
Licensed to practice medicine in the State of Florida
Current DEA number
Eligible for malpractice coverage under company policy terms
Board eligible or board certified in a recognized field of the American Board of Medical Specialties
Knowledge and experience in palliative medical practice and symptom management
Basic computer skills including use of email and word processing (Microsoft Outlook and Word or equivalent programs)
Valid driver's license and automobile insurance in the State of Florida; must have reliable transportation to be able to meet visit schedule
Must be able to physically access any home in any LifePath Hospice area, i.e., navigate stairs, and narrow spaces, tolerate heat and lack of air conditioning.
Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
Plan to become certified by the American Board of Medical Specialties in Hospice and Palliative Medicine when eligible if not already certified
Review patient admission prognostic indicators to determine hospice appropriateness at time of admission
Work cooperatively with the Admissions Department to facilitate timely provision of services to potential company patients, including performing hospice evaluation visits for potential patients
Serve as a team physician for one or more of the interdisciplinary teams (IDT) as assigned by the Medical Director
Provide clinical expertise and participate in interdisciplinary team decisions regarding diagnosis and medical management of patient/family needs, assuring that the highest quality of medical care is provided and that continuity and comprehensiveness of care is maintained in a cost-effective manner.
Consult to patients' primary and attending physicians regarding integration of patients' hospice care and primary or other care
Validate patient/family plans of care in coordination with a patient's attending physician
Provide medical coverage for patients without attending physicians.
Develop program to assume primary or consultative care of patients admitted to the company whose referring physician desires such services
Care for patients in the company's inpatient facility, as designated by the Medical Director.
Make home visits to care for patients to meet assigned productivity goals.
Teach designated in-services to company staff
Develop and recommend written guidelines for symptom control to company committees
Supervise and educate fellows, residents, and medical students participating in company programs
Support or conduct research to improve hospice care
Contribute to research efforts of the company by actively recruiting research subjects
Conduct educational seminars, in-services and presentations for physicians, nurses and other health care personnel whose support for and understanding of the hospice program is an integral part of assuring that the company services are made accessible to patients and families
Assist in developing clinical practice standards for physicians and nurses in the practice of hospice care
Assist in developing unique roles for non-physician practitioners (ARNP, CNS, PA, etc.) in order to expand medical services in a cost-efficient manner.
Prepare patient billing information in conjunction with the appropriate billing agency
Complete timely medical documentation of all patients seen who are cared for by physician
Assist in improving quality of medical documentation by company staff
Contribute to Performance Improvement efforts of medical documentation by company staff
Promote company programs to nursing homes, ALFs, HMO's, hospitals, and other referral sources
Attend company Medical Staff meetings regularly.
Participate in relevant company committees*
Represent company at appropriate local, state, national, and international forums*
Work cooperatively with the Corporate Compliance Officer as indicated
Participate in other clinical/administrative responsibilities as directed by the Medical Director*
Work cooperatively with and reports to the Medical Director to ensure quality care delivery to all company patients
Maintain a regular schedule of medical participation in all components of the company's care program, and maintain 24 hour, 7 day a week coverage of the company's program
Provide coverage in the event of absence
Other duties as assigned by the Medical Director.
Medical Director Qualifications and Responsibilities:
MD or DO from an accredited medical school
Licensed to practice medicine in the State of Florida
Board certification by American Board of Medical Specialties (ABMS) in a recognized specialty
Hospice Medical Director Certified required or within 1 year of hire
Certification in Hospice and Palliative Medicine preferred
Minimum two years' previous hospice or palliative medicine experience
Previous medical administrative leadership experience
Expert verbal and written communication skills and public speaking presentation ability
Leadership skills to supervise and manage medical staff
Demonstrated skills in fiscal management and budget
Skilled in effective conflict resolution
Able to manage multiple priorities simultaneously
Valid Florida driver's license and automobile insurance per Company policy
Ability to make home/residence visits

Coordinate the work of medical staff, chair medical staff meetings, and supervise educational plans of fellows, residents and students who rotate at the hospice.
Assist in the development of written guidelines for symptom control and recommend such guidelines to appropriate committees.
Provide authorization of non-formulary medications for medical staff.
Enforce care consistent with patient/family care policies and procedures.
Monitor and advise medical staff of cost effective prescribing habits.
Participate in medical reviews, as needed, and directed by the AVP, Medical Services/Chief Medical Officer.
Respond to issues identified in Ethics Point in a timely manner, develop an action plan, and provide education/implement changes, as needed, based on findings.
Participate and lead medical staff quality assurance/performance improvement program.
Maintain a regular schedule of medical participation in all components of the hospice care program, and maintain 24 hour, 7 day per week medical staff coverage.
Provide supervision including feedback, both positive and negative, as well as performance reviews to the medical staff that reports to the Medical Director.
Provide clinical expertise and participate in interdisciplinary team decisions regarding diagnosis and medical management of patient/family needs, assuring that the highest quality of medical care is provided and that continuity and comprehensiveness of care is maintained.
Provide suggestions to the IDG that foster high quality cost effective care for all patients.
Provide or coordinate direct patient hospice care through home visits, clinic visits, or inpatient care, as appropriate.
Ensure a member of the medical staff reviews the admission history and physical examination for each patient, including all available hospitalizations, office notes, laboratory data, etc.
Provide or ensure that a medical staff member will provide medical validation of the plan of care for each patient/family with the coordination of the patient's attending physician and medical staff.
Ensure the medical validation of the estimated prognosis of the patient as provided by the patient's attending physician.
Participate in decisions regarding appropriateness of admissions, as necessary.
Act as a consultant to patient's physician and other members of the interdisciplinary team, as needed.
Act as a liaison with patient's primary or attending physician. The primary physician is encouraged to provide primary care to his/her patient while the patient receives hospice care.
Ensure that each new medical staff receives appropriate orientation to the organization and are competent to serve in their medical staff position role.
Provide medical coverage or ensure medical staff coverage for each patient who is deemed appropriate and who has no attending physician.
Assist in improving the quality of medical documentation by the medical staff and ensure timely completion.
Assist in securing medical records to better determine appropriate medical care for patients.
Provide coverage in the event of absence of other medical staff coverage.
Supervise the hospice medical staff including advance practice nurses in their respective service area.???????
Contact practicing physicians to introduce the hospice; encourage referrals, answer medical and other physician concerns regarding such care.
Introduce the hospice program to nursing homes, health maintenance organizations, hospitals, and other potential referral sources.
Present educational offerings to physicians, nurses and other health care personnel whose support is necessary to ensure hospice services are accessible to all patient/families who need them.
Provide updates and ensure medical staff practices are consistent with organizational protocols and guidelines.
Provide ongoing education to medical staff regarding determination of related or unrelated medications, procedures and hospitalizations as well as determination of hospice eligibility.
Participate in the ongoing in-service education of the hospice medical and clinical staff.
Actively participate in research to improve hospice care, as appropriate.
