Swedish Medical Group is looking for a Practice Coach \(Full Time, Day Shift\) to work in the Aco Department in the Bank of America Building in Seattle, WA\.
The Practice Coach works with our Swedish Medical Group clinics and is the on\-site support individual who specializes in assisting clinic staff and teams through times of increased change and transition\. They collaborate closely with the clinics to facilitate strategic implementations using change management skills, team development and lean methodologies while improving quality health outcomes and increasing the efficiency of the practice\. Their primary function is to navigate staff towards successful completionof all medical home initiatives and to coach managers, providers and staff, to develop the capacity for sustained change and improvement\.
The Practice Coach is part of the Patient Centered Medical Home \(PCMH\) team which supports Primary Care SMG clinics\. In this role the Practice Coach works closely with the clinic manager, medical director, supervisors, providers and staff\. The position collaborates with other program coordinators and managers to develop implementation and operational plans related to clinic improvement initiatives\.
In this position you will:
Facilitate SMG primary care practices in the adoption and operation of the NCQA Patient Centered Medical Home model
Work directly with clinic managers, providers and staff to identify and address issues related to maintaining an effective and efficient medical home practice
Help to prepare the organizational infrastructure for quality improvement implementation Create ad\-hoc reports on quality measures and use assessment tools to assist a practice's continuous improvement
Actively participate/facilitate program steering committee meetings and clinic\-based Process Improvement Team meetings
Assist in implementation of quality improvement, Lean, and other practice improvement activities
Provide clinic leadership and staff training related to implementation of the Swedish Medical Home Model
Help clinic management define actionable goals and plan small scale tests of change
Educate clinic management about the Swedish Medical Home and implement model elements based on each clinics unique context
Foster a culture of continuous improvement that includes the use of performance data to understand and increase the reliability and effectiveness of care
Create capacity for managing the care of a population and growing a culture of population health
Assist and monitor clinics in their ability to implement and maintain new processes
Serve as a liaison between the clinics and senior leadership
Critically evaluate self\-performance as reflected in the performance of practices s/he is coaching
Work out in the field approximately 75% of the time
Requiredqualifications for this position include:
Bachelor's Degree OR minimum 6 years' experience required in healthcare setting
Familiarity with continuous quality improvement methods \(Transitions, LEAN, SixSigma, etc\.\)
Demonstrated effectiveness in facilitation of teams and familiarity with group dynamics and communication
Preferredqualifications for this position include:
Three \(3\) plus years' work experience in an ambulatory health care setting strongly preferred
Prior experience working in a large health system preferred
Prior experience with EPIC ambulatory, including knowledge of data entry and reporting
Prior experience in practice transformation/ NCQA Patient Centered Medical Home preferred
Prior experience working with a diverse population
Familiarity with organizational behavior, change management facilitation, and practice coaching preferred
Experience working with Senior Leaders across multiple settings
